Mission Statement
Outreach
has the purpose of educating the rural community about HIV risk, behavior
change, opportunity and availability of HIV testing and counseling, client
advocacy and availability of services to HIV+ residents. Case management
is the partnership between the client and the Case Manager in which the
involved parties work together to maintain client independence to the
greatest extent possible by increasing client access to appropriate services
in a timely, coordinated manner.
HIV Prevention Education -
Small group educational presentations on how to prevent HIV, offered free
to jails, restitution centers, probation departments, treatment programs
and other facilities.
Outreach - Street outreach
to individuals offering safer sex and drug use information
Case Management (Linkage with
medical and social service providers, short and long term planning) for
HIV+ residents of Bastrop, Blanco, Burnet, Caldwell, Fayette, Hays, Lee,
Llano and Williamson Counties.
· Transportation to medical and dental appointments
· Financial assistance for prescriptions, radiology services, specialty
physician services, mental health counseling and rent/utilities.
· Emergency food vouchers for low-income clients.
